[CK] Vancomycin trough(prior to 30mins to 1 hr) - RENAL

1.You should check prior to the 4th or 5th dose.
2. Initial/change dose -> recheck every 4th dose but if you really concern, can check even earlier with discussion with pharmacy.
3.Renal function change/UOP change/nephrotoxic meds => recheck
4.Long term => once a week check
